I've been using these 2 excellent 'Contact Forms' for a while.

www.foxyform.com and www.emailmeform.com

FoxyForm is a simple but prebuilt fields. Just one step to integrate with our site. Whereas emailmeform is a customizable one.

When a visitor submits his contact information and message, it's directly delivered to our email.

I'm using them as a free member.

I've built my own contact form that saves the message to a database instead of sending me an email.. :)
Still haven't found the time to add my captcha to it though.. but I can't remember when was the last time I got spammed.. :P
